What is the Leap Program?

At Spark Home Health, we understand the path traveled by both parents and caregivers when their precious little ones begin life’s journey in the NICU can be difficult. For some a NICU stay is an anticipated event, for others, it’s a surprise change affecting the birth plan and the length of the baby’s hospital stay. During the NICU stay, an exceptional clinical team provides support and care for the baby and family. While finally being able to bring a baby home is exciting, it can also be worrisome to be without the highly experienced team of hospital physicians and nurses. The Spark Home Health Leap team seeks to fully support the move home for babies and families leaving the NICU.

What Services Do We Offer?

A team of highly qualified pediatric nurses, physical therapists, occupational therapists, speech-language pathologists and lactation consultants work to make sure the family’s first few months at home with their little one is successful by focusing on:

  • Ongoing medical care

  • Feeding

  • Sleep

  • Developmental milestones

  • Family attachment and bonding

Our Leap Team Supports Each Family by:

  • Care coordination between providers and the family

  • Meeting with the family and hospital team before discharge

  • Helping to prepare for the baby’s arrival home

  • Assessing and evaluating a baby’s ongoing medical needs shortly after hospital discharge

  • Helping to identify a support network including outside community resources

  • Maintaining regularly scheduled appointments to ensure each child’s best health and development for as long as medically necessary
